Tele-Counseling/Video Sessions

I now provide HIPAA compliant secure live video sessions! If you find that you are unable to attend your appointment, or if you would like to schedule a video appointment, you can contact my office and arrange your session to be a tele-counseling/video session.  My patient forms listed on the Documents page can be obtained by clicking on the Personal link at the top of the page on my tele-health video page which allows you to print out and complete your intake form, mood rating forms, a history form, a health form and your professional disclosure and HIPPA forms and then email them to me prior to your next session if they have not already been completed.  You can also mail them to me as well or fax them to me at 281-444-0368.

Payment for my tele-health video services counseling may be made on the website.  Click on the Payment button and make payment through the patient portal.  You may also contact my office manager, Anissa Weiss, and give her your payment information so that payment can be processed via our credit card processing center in our office.  Your information will be kept confidential and you will receive a confirmation email stating that your payment has been received. Please note that a separate tele-health disclosure and client permission form may be downloaded and signed and returned to my office.

Please note that any tele-health counseling must be conducted in a private environment that is condusive to your participating in this service.  Please also note all aspects of the tele-health counseling process are confidential.

Before your Live Video Session please check the following.

1. Complete initial setup of your account by clicking the link above and following the prompts in the email you receive.

2. Make sure you have a webcam set up on a laptop or desktop PC or Mac

3. Make sure you have a stable internet connection and have Chrome or Firefox downloaded.
