Current Fee Information
I will be happy to sit down with you and discuss my session fees at the time of our initial session. I provide my services at my 1960 northwest Houston location on Mondays, Tuesdays and Thursdays. I provide my services at my Woodlands office location on Wednesdays, Fridays and Saturdays. Please specify the office location that you would like to receive my services at.
My self pay out of pocket rate for my therapy services is $65.00 and my fee for an AD/HD evaluation is $250 total and a chemical dependency evaluation (non-SAP) is $200. The DOT SAP evaluation fee is $275.00.
I accept most insurances and employee assistance programs that are offered by your employer's company. Many times, your insurance will have you pay a small co-payment of $15-$25 per session. There is no co-payment for employee assistance program sessions provided through your company. My standard rates for various types of evaluations are reasonable and cost effective. My standard rate for a Substance Abuse Professional evaluation is $250.00 total for both the initial and return to work evaluation sessions combined. I accept credit cards, debit cards, cash and personal and company checks as forms of payment for the services that I render.
Insurance Billing Service
We utilize an insurance billing service to file our insurance claims for the services that our clients receive from Dr. Garlock. Our billing service is Claims Connection and it is operated by Mrs. Karolyn Michaels. If you have any billing questions or concerns, she may be reached at 800-775-1585 between the hours of 9:00am to 5:00pm eastern standard time.
Insurance
My services may be covered in full or in part by your health insurance or employee benefit plan. My office manager, Mrs. Anissa Vasquez, can check your insurance coverage and/or your employee assistance program coverage to insure that your benefits are available. If you like, you may also check your coverage by calling the telephone number on the back of your insurance card and carefully asking the following questions:

What is my deductible and has it been met?

How many sessions per calendar year does my plan cover?

How much does my plan cover for an out-of-network provider?

What is the coverage amount per therapy session?

Is approval required from my primary care physician?
Reduced Fee
Reduced fee services are available on a limited basis according to financial hardship. Dr. Garlock will complete a financial hardship and fee reduction form to determine what fee will be charged for his services.
Payment
We accept most credit cards, debit cards medical account cards, cash and personal and company checks as payment for your services. Dr. Garlock has a credit card processor that allows him to immediately process debit and credit card charges as payment for the services that he renders to his clients. He can accept credit cards or debit cards or employer medical credit account cards for payment purposes. He does not accept American Express cards for payment purposes. He accepts insurance and employee assistance programs as forms of acceptable payment for his rendered services.
We also accept most Medical Savings Account cards, Flexible Spending Account cards, Health Savings Account cards, and Health Reimbursement Account cards. You can use these account cards at no out of pocket cost to you. We will be happy to check on these accounts to verify that we can take this payment for the services that Dr. Garlock has rendered.

Contact our office to what amount your insurance or employee assistance program may pay towards the cost of your services. Not every company has an employee assistance program (EAP) and the employee's company will need to be contacted to determine if there are any employee assistance program benefits available at their company.
Cancellation Policy
If you do not show up for your scheduled appointment, and you have not notified us at least 24 hours in advance, a no show fee of $35.00 may be charged for the missed appointment.
