Dr. Garlock is a Substance Abuse Professional and meets the requirements of the Department Of Transportation to provide substance abuse evaluation and referral services that allow individuals who have experienced a violation of the Department Of Transportation rules and guidelines to complete the return to work requirements as required by the Department Of Transportation.  This includes truck drivers, package car drivers, pilots, ship captains, pipeline workers, and others who are regulated by the Department Of Transportation rules and guidelines.  He charges $275.00 for the entire SAP process fee.

Dr. Garlock charges $275.00 total for the DOT SAP process.  This includes both the initial evaluation session and the return to work session.  Normally, we will schedule you immediately and conduct the initial evaluation session so that you can begin the return to work process.  Dr. Garlock will conduct a chemical dependency evaluation and make a fair and reasonable recommendation for your required services that are acceptable to the Department Of Transportation.  Dr. Garlock can recommend any of the following depending upon your current evaluation and service needs:  (1) eight hours of alcohol and drug education; (2) the eight hours of alcohol and drug education along with eight free 12 step self help meetings and (3) treatment for alcohol or drug abuse and dependency.  Each individual is given a fair and impartial evaluation and the recommendations are discussed with each individual who is receiving this service.  Most often, education is recommended if an individual does not have a patterned alcohol or drug abuse problem.  Please note that Dr. Garlock cannot provide the recommended services but he will provide you with the name of two low cost options to obtain your services near your home.
